Dancing Sky Area Agency on Aging seeking a 32 hr/wk. FTE Minnesota Aging Pathways Resource Coordinator.

This position will be responsible for planning intensive care coordination for people wanting to remain in thome or relocate to the community after a stay in a care facility. Minimum of a bachelors degree from an accredited college or university in Nursing, Social Work, Gerontology, or a related human services field.

Experience working in person-centered discharge planning, charting, care coordination, or related tasks. Proficient in problem-solving skills, adept at phone etiquette, strong in computer skills, and have an independent work style. Applicants who are within 40 miles of Warren will be required to work in the Warren office a minimum of 3 days a week. If outside a 40-mile radius, work from home status will be granted. Regular travel is required, including throughout the 21-county service area.

Salary Range $54,233 -$65,000 plus benefits. Program Specialist 2, Grade 4. Hiring Salary $62,000 to $64,000.
